Our Specialties

Four specialties, delivered as one care plan.

Each specialty below is available as a standalone consult service or as part of an integrated care plan coordinated with your facility's nursing staff and each resident's primary care provider.

01  —  Behavioral Health

Geriatric Psychiatry

Behavioral and mood changes are common in long-term care, but they're rarely simple. Our geriatric psychiatrists evaluate residents on-site, distinguishing between depression, anxiety, delirium, and dementia-related behavioral disturbance — and build medication plans suited to aging physiology and existing prescriptions.

Conditions we manage:

  • Depression and anxiety disorders
  • Dementia-related behavioral and psychological symptoms
  • Delirium evaluation and management
  • Psychotropic medication review and simplification
  • Decision-making capacity evaluations

02  —  Neurologic Care

Neurology

Post-stroke deficits, seizure disorders, and progressive neurologic conditions require consistent, on-site follow-up — not a single hospital consult. Our neurology team manages residents longitudinally, adjusting treatment as conditions evolve and coordinating closely with rehabilitation and nursing staff.

Conditions we manage:

  • Post-stroke care and recovery monitoring
  • Seizure disorders and anti-epileptic management
  • Peripheral neuropathy
  • Parkinson's disease and other movement disorders
  • Cognitive decline and dementia workup

03  —  Pain Medicine

Pain Management

Unmanaged pain drives agitation, falls, and hospital transfers. Our pain management providers build structured, evidence-based plans tailored to each resident — with a deliberate bias toward non-opioid options where appropriate, and close monitoring where opioids are already in use.

Conditions we manage:

  • Chronic musculoskeletal and neuropathic pain
  • Post-surgical and post-fracture pain
  • Opioid stewardship and tapering support
  • Non-pharmacologic and interventional options
  • Pain-related behavioral escalation

04  —  Serious Illness Support

Palliative Medicine

For residents living with serious or advancing illness, our palliative medicine team focuses on comfort, dignity, and clarity — managing symptoms, supporting families through difficult decisions, and coordinating goals-of-care conversations alongside the facility and each resident's primary care provider.

How we help:

  • Symptom management for serious and chronic illness
  • Goals-of-care and advance care planning conversations
  • Family and caregiver support
  • Coordination with hospice when appropriate
  • Support through care transitions
